  • What should I bring to my first appointment and thereafter as appropriate?
    • A Driver’s License or state identification card
    • The name and number of an emergency contact person
    • Complete and current health insurance information including an insurance card for each insurance company with which you have coverage
    • Payment for deductibles, copays and coinsurance–Payment options include cash, checks, and the following credit cards: Visa, MasterCard, American Express, and Discover
    • Appropriate paperwork or pre-certification numbers from your PCP or health plan for referrals and authorizations
    • Any completed patient information forms that may have been sent to you by our office
    • Any other information you think is appropriate to the visit

     

    Insurance General Information: For patients with accepted medical insurance, the clinic will bill the insurance company directly. It is the patient’s responsibility to confirm payment of the claim with the insurance company.Patients need to verify their health plan coverage and benefits prior to their visit by calling their insurance carrier or employer with any questions they may have. If you need to know if a specific physician is a participating provider with your health plan, contact your health plan for this information.Often your insurance company will require you to complete certain forms such as accident forms, student status forms, etc. It is the patient’s responsibility to see that all necessary forms are completed as required by their health plan. The patient must understand and comply with all of their insurance plan requirements. Payment for your bill is ultimately your responsibility.

  • Medicare Information: Our physicians accept Medicare assignment. This means that we do not charge the patient over Medicare’s allowed amount. The patient is responsible for deductibles, coinsurance, and copays as determined by the Medicare Program. If you have coverage through a Medicare Replacement Plan, be prepared to show your insurance cards for both Medicare and the Medicare Replacement Plan.Any referrals needed for the Medicare Replacement Plan are the patient’s responsibility to obtain. Please check to see if the physician you are planning to see is a participating provider with your specific Medicare Replacement Plan.There may be times when a service provided to you is not covered by your Medicare plan. Should this occur, we will ask you to sign an Advanced Beneficiary Notice and we will collect payment for the non-covered service at the time of the visit.
  • Medicaid Information: Our physicians accept Medicaid patients. However, not all of our physicians are participating providers with all Medicaid Replacement plans or Medicaid HMO plans.If you have coverage through a Medicaid Replacement or Medicaid HMO Plan, be prepared to show your insurance cards for both Medicaid and the Medicaid Replacement (HMO) Plan as applicable. You need to make sure you have all the necessary referrals that are required by your PCP and/or the Medicaid plan.The patient is responsible for any copays due at the time of service. There may be times when a service provided to you is not covered by your Medicaid plan. Should this occur, we will ask you to sign the appropriate Medicaid form(s) and we will collect payment for the non-covered service at the time of the visit.
  • Billing/Statements: Our statement is only for the Fondren Orthopedic Group physician(s) and their ancillary services, and is separate from any bill you may receive from other doctors, hospitals, or other in-patient or out-patient facilities. The statements from us may reflect accounts billed to your health plan that are not yet resolved, or balances that you owe.To call and discuss a bill or a statement you have received from the Fondren Orthopedic Group L.L.P. please call our main number at 713-799-2300 and they will connect you to the appropriate billing specialist or the appropriate financial counselor. Our billing staff can be reached Monday through Friday from 8:30 AM to 4:00 PM except holidays.  To call the Texas Orthopedic Hospital (TOH) to discuss their bills or statements please call 866-783-4549 or visit the TOH Website.
  • No Insurance (Self Pay): Please call our main number at 713-799-2300 and you will be connected to a financial counselor. At this time the financial counselor may be able to suggest funding options that may be available to you such as Texas Medicaid, the Department of Assisted Rehabilitative Services (DARS), the Texas’ Crime Victims’ Funds, etc. They will also be able to help you set up an appropriate payment plan.
  • Online Credit Card Payments: Online credit card payments are available. It is easy to use this online system, as well as being a very safe and secure transaction. Click here to make a payment online. After we process your payment we protect your credit card number so it can’t be seen by anyone.
