- Referral paperwork: Your insurance company can tell you if a referral is needed before being seen by our practice. If so, bring the letter from your referring physician to your appointment.
- Patient forms: Download our patient forms to print out, complete and bring along with you. This information will be essential to help our team determine the right treatment plan.
- Photo ID, payment method and insurance card: Bring your driver’s license or another form of ID as well as a payment method and your health insurance card. Note that your insurance may also request additional information, like accident forms.
- List of questions for your doctor: It’s completely normal to have questions or concerns about your treatment. We recommend bringing this list of questions to your appointment, and our team will be happy to answer them.
